Payment Policy:
1. We consider our fees to be fair and
reasonable. Your insurance company may not consider
our charge at 100%. We do not make adjustments for differences
not covered by individual insurance policies.
2. We participate with a select group of health insurance
plans in which we will file claims directly to the insurance
carrier. If it is an insurance that we do not file,
we will make every effort to assist you with the reports,
statements, etc. needed for filing your own claims.
Payment in full is expected at the time of service (this
includes any deductibles and/or co-payments).
3. If you are a Medicare patient, you will receive an
“Explanation of Benefits” from Medicare
showing how much they paid toward your bill. If you
have a secondary insurance, we will make every attempt
to file the claim with your insurance as a courtesy
for you. However, you will still be responsible for
payment of your deductible, 20% co-payment, and/or non-covered
charges.
4. As part of good patient care, our physicians recommend
yearly routine physicals. Not all insurance plans will
cover this benefit. It is your responsibility to check
with your insurance company prior to your visit as to
whether it will be covered. If not, you will be expected
to pay in full.
5. While we are happy to provide treatment for accident
victims, we do not make any exception for them in our
payment policy.
